Patient Health Log is a Palm medical software program made for tracking
- Patient Information - name, address, city, state, zip, phone, dob, weight, height and blood type.
- Medications - Allows you to track the medications your taking, how much , when, when to re-order, re-order number and pharmacy, date of last refill.
- Vital signs - Allows you to track temperature, weight, blood pressure and pulse.
- Diabetes - Allows you to track blood sugars, insulin dosages, meals which includes calories, fats, sodium, carbohydrates and protein.
- Symptoms - Allows you to input the symptoms you having and to make notes about what might have caused them to show the doctor later.
- Fluids - Is for tracking fluids in and out This is good for dialysis patients and transplant patients.
- Labs - Is for putting all you labs in one place for reference. If you have medical conditions that you take regular labs this is a good way to keep up on how your doing.
- Doctors - This just links to your Palm Address Book.
- Appointments - This links to the Palm Date Book.
- Medical History - This takes you to another menu which has surgeries, health checks, vaccinations, diagnosis and allergies, contacts and insurance information.
- Surgeries - Track your surgeries and note any complications.
- Health Checks - These are the common health checks that a diabetic would have, it makes it easy to so when the last eye exam was or foot exam A1C, it also has cholesterol, Total, LDL, HDL.
- Allergies - List your medication allergies and symptoms.
- Vaccinations - Track Vaccinations and when you had them last.
- Diagnosis - Track the date and diagnosis of any problems or disease.
- Contacts - I admit this was not a good place for this but space made it impossible to put where I wanted it. This is where you add any contact in case of emergency.
- Insurance - Enter your Insurance information.
